What to bring, step by step
For a new or first-time U.S. passport. Bring the items below and we hand-carry and expedite your application.
You can apply for a first-time passport if
- You are applying for your first U.S. passport.
- Your previous passport was issued when you were under age 16.
- Your previous passport was lost, stolen, or damaged.
- Your previous passport was issued more than 15 years ago.
- Your name changed since it was issued and you cannot legally document the change.
Complete the DS-11 application form
The passport office only accepts DS-11 forms completed online. Fill it out completely, including your travel details and emergency contact, review it, and print 2 copies. Do not sign it until an acceptance agent or county court clerk instructs you to.
Two authorization letters
Complete two authorization letters, check the 2 option boxes you prefer, and sign and date both copies. The courier name "Premier Passports" at the bottom must be handwritten. Hand one letter to the post office and the second letter to us.
Proof of U.S. citizenship
Prove citizenship with any one of the following originals:
- Certified U.S. birth certificate issued by the city, county or state, showing the applicant and parents.
- Previous expiring or expired U.S. passport, undamaged, issued less than 15 years ago.
- Certificate of Citizenship or Naturalization.
- Consular Report of Birth Abroad or Certification of Birth.
Proof of identity
A clear copy, front and back, of your valid driver's license or state ID. If you are recognizable, you may also use a current government ID (city, state or federal) or a current military ID.
Proof of departure
A copy of your round-trip ticket (e-ticket) from the travel agency or airline. To expedite, your travel date should be within 2 weeks of the application date.
Photographs
Two color 2x2 inch photos on a white background: full face, no glasses, not smiling, no shadows, both eyes open, with the chin-to-forehead measurement between 1 and 1 1/4 inches. We take compliant photos in our office.
Government fee
$212.05, paid by check or money order payable to the U.S. Department of State. If you also want a passport card, the total fee is $242.05.
Acceptance facility & execution
Present the items above to the authorized agent at an acceptance facility near you and let them know you are using an expediting service to hand-carry your application. Pay the $35.00 execution fee to the agent. The agent seals the envelope, marks it "To Be Opened by U.S. Passport Office Only," and returns it to you. Do not open it. It is time sensitive and must reach the passport office within 5 days.
Then send these to us by FedEx
- The sealed government envelope, with the applicant's name and date of birth written on it (submit within 5 days of execution).
- A copy of your completed DS-11 application form.
- Clear copies of your valid driver license, front and back.
- The letter of authorization with your original signature.
- Payment of the service fee for your speed, by money order or cashier's check payable to Premier Passports. Add return shipping unless you include a prepaid FedEx air-bill or pick up in person.
- A completed and signed Premier Passports order form. Our FedEx shipping address is on the order form.
