Valid passport undamaged, signed, 1 blank page, valid 6 months from departure.
Copy of valid ID with a matching address, or a major utility bill.
Birth certificate original and copy, notarized and then legalized by the consulate.

- Birth certificate, both parents' passport or license copies, and a notarized consent if not traveling with both parents.
Business letter
- On company letterhead with an original signature, stating your name, position, nature of work, the number of days, where you are staying and who provides the financial guarantee. The Indonesia contact must be named in the letter.
Bank statement
- A copy of your most recent statement.
Flight itinerary
- Include a copy if you have one. It is okay if you do not.
Everything in the business visa
- Including the company letter and bank statement.
Invitation letter
- On the company's letterhead with the company seal or the red foreign ministry seal. Original or scanned.
Copy of hotel reservation
- A copy of your hotel reservation.
